Medical billing and payment procedures are often complex and difficult to understand for patients and their families. Most patients are covered by some form of group health insurance that will pay a large percentage of the cost of your care. When you provide us with current and accurate insurance information, we can successfully file most insurance claims on your behalf. If not mailed ahead of time, this critical information will be collected and reviewed at the beginning of your first appointment. In addition to any private insurance carrier information, patients covered under Workers Compensation insurance should present all information and records associated with a work-related injury and claim, including the claim number, Workers Compensation insurance carrier name, policy number (usually a social security number), and date of work-related injury. Some of the costs of your care, such as co-payments and policy deductibles, may not be covered by your group insurance plan. We ask that you remit payment of any co-payment due at time of your initial appointment. Policy deductibles will immediately become the responsibility of the patient, once we are so notified by your insurance carrier.If your visit is covered, your insurance company may still require that you (pay) an annual deductible or percentage of the costs. Patients with no current health care insurance will be expected to remit up to $250 of your initial visit costs at the time of the first visit. Immediately following your initial visit, we will meet with you to estimate your total treatment costs, establish a mutually acceptable payment plan and sign a promissory note. Portland Foot & Ankles policy calls for payment in full within twelve months from the initial visit with a minimum monthly payment of $50. |
