On your first visit to Portland Foot & Ankle you will be asked to provide us with personal health information. This information is needed to provide you the best possible care. At each subsequent visit we will record information regarding your exam, treatment and any changes in your personal information or health insurance. It is Portland Foot & Ankle’s policy that personal health information is confidential and privileged information. We will only share your health information with individuals, doctors, or other agencies you have authorized to have access. Please be advised there are some federal government agencies that have access to medical records without prior patient authorization.

Insurance and Payment Information

We appreciate your payment at the time of your visit unless prior payment arrangements have been made. We require payment of your co-insurance or co-payment at the time of your visit. We will, however, submit claim forms for all covered services to your insurance company on your behalf. If your visit is covered, your insurance company may still require that you pan an annual deductible or percentage of the costs. Therefore, after we receive payment from your insurance company, you may still receive a bill from us for the amounts not covered. We appreciate prompt payment. Cash, personal checks, Visa, MasterCard or Discover are all acceptable forms of payment. Any billing inquires should be directed to our billing department Monday-Friday from 8:00AM to 3:00PM at 207.879.1339, choose option 2 from the auto attendant.


Payment Plans

For patients demonstrating financial hardship, a payment plan may be arranged for the balance of the bill not covered by your insurance company.


Managed Care (HMO) Prior Authorizations

If you are enrolled in a managed care insurance plan (HMO) it is your responsibility to obtain a preauthorization or written referral from your primary care physician (PCP) regarding your condition before seeing your podiatrist. Often such insurance plans require that your PCP decide which sub-specialist you should see. While at times inconvenient, your insurance plan and your PCP have entered into an agreement to ensure that your PCP provides a comprehensive plan for managing all of your medical conditions.

In order to receive your full insurance benefits, you should review your insurance policy to see if preauthorization or second opinions are required prior to surgery or testing.


Custom Molded Orthoses

When ordering custom molded orthoses through Portland Foot & Ankle, a 50% deposit is required upon casting and the remaining 50% due at time of being dispensed. It is rare and only for particular medical conditions that custom molded orthoses may be covered by medical insurance.
